Scott Crutchfield, PT, DPT

Scott graduated with his DPT from Western University of Health Sciences in 2016 with a primary focus on manual therapy and Orthopedics. Scott has 10 plus years of experience in strength and conditioning. He works in all age groups and skill levels with a main focus on professional Hockey, Lacrosse and Skiing athletes. In addition to his role as a physical therapist at the clinic, Scott also serves as the clinic director. Scott grew up in Toronto, Canada and still has a love for competing in hockey as well as golf, cycling and downhill skiing. When he isn't riding his bike or playing hockey, he enjoys spending time with his wife and dog.
Tenay Maddox, PT, DPT

Tenay has been in practice for 7 years, having worked in a staff therapist capacity as well as an in-home orthopedic setting most recently. Tenay graduated from Azusa Pacific University in 2009 and has had a focus on manual therapy in her multiple settings since graduation. Tenay enjoys spending time with her first born bundle of joy and her husband traveling, taking cruises, playing the occasional round of golf with her husband and rooting on the Angels!
Patrick McGinnis, PT, DPT

Dr McGinnis graduated with his Doctorate in Physical Therapy from Azusa
Pacific University in 2018. In addition to working at Cal Rehab, he is
currently in the Orthopedic Residency at Kaiser Baldwin Park. When not
working, Patrick enjoys going for new deadlifting PR's, playing
basketball, rock climbing and reading a good book.
Takuma Shito, DPT, OCS, CSCS
Takuma obtained his Doctor of Physical Therapy degree in 2016 from Azusa Pacific University. He then went on to complete his orthopaedic residency at Casa Colina Centers for Hospitals and Centers for Healthcare in 2019, obtained his Orthopaedic Clinical Specialist certification that same year. He is also a NSCA Certified Strength and Conditioning Specialist since 2015. He has extensive experiences in post-op rehabilitation from his experiences working at a physician owned practice, as well as from his experiences at Casa Colina. Takuma enjoys weightlifting regularly and listening to oldies music.
Nick Cvjetkovic, OTR/L, ATC

Nicholas graduated with a Masters of Science in Occupational Therapy
from Stanbridge University. Nicholas found his passion for hand therapy
while volunteering with California Hand Therapy and began working for
the company following graduation. He attended California State
University, Long Beach where he earned his Bachelors of Science in
Athletic Training. In his free time you can find him spending time with
time with family, friends, and is an active member of the Croatian
community in Southern California.
Jill Murch, COTA

Jill has been in practice for over 20 years and enjoys treating upper extremity patients as well. She enjoys baking and cheering her youngest daughter in volleyball matches. She also enjoys spending time with her husband and 3 other children as well.
